I certify that my answers are true and complete to the best of my knowledge. If this application leads to employment, I understand that false or misleading information in my application or interview shall be grounds for dismissal.

I authorize Little Harbor Club to request and obtain verification that the information given by me on the application is true, accurate and complete. I understand that such verification may include, but may not be limited to, background information pertinent to the position for which I have applied, verification of education, verification of employment history, investigation of criminal history, etc.

I understand that Little Harbor Club will utilize an outside firm or firms to assist it in checking such information, and I will be provided opportunity authorize such an investigation by information services and outside entities of the company's choice. I also understand that I may withhold my permission and that in such a case, no investigation will be done, and my application for employment will not be processed further.

I authorize my current and all previous employers to cooperate with Little Harbor Club and to release, on a confidential basis, any information they may have concerning me. I specifically release from liability any current or former employers, their agents, representatives, employees, officers, directors, etc. for or on account of their providing/disclosing such information to Little Harbor Club.

I agree that if hired I will comply with all the rules and regulations of the Little Harbor Club.

I understand and agree that my employment and compensation are for no definite period and may, regardless of the time and manner of payment of my wages and salary, be terminated at any time by me or the Company, with or without cause, and with or without any previous notice.

The Immigration Reform and Control Act of 1986 states that employers must require all persons hired to submit documents to the employer showing their identity and their right to be lawfully employed in the United States. It also requires that the employee complete and sign a government form to this effect. I understand that if hired by Little Harbor Club, I will furnish, within three days of starting work, original documents for inspection that verify my identity and that I am legally permitted to work in the United States. Furthermore, I understand that my employment will be terminated if I fail to provide the necessary documents.
